Medical release forms

Just an FYI: You need to have a new Medical Release and Consent form for 2011-12. You can either contact me or click the link to the right. Thanks! If your student is thinking about going to camp, this form must be filled out and turned in. It's up to you whether you get it notarized or not. We will take care of it if you don't.
